Will insurance pay for the medicine?

Tell the plan the exact product name and why it is being prescribed. Ask for a written coverage answer.

Find the plan’s medicine list and approval step

A plan’s formulary lists the medicines it covers. Prior authorization is the plan agreeing beforehand to pay.

As of September 26, 2026, a Medicare medicine that may be covered for a condition other than weight loss still has to meet the plan’s list and approval requirements. A

Possible coverage is not a promise of payment. A

Ask in your own words

Is this product covered for my diagnosis, and what approval steps come first?

Ordinary Medicare drug coverage excludes weight-loss use

Part D is Medicare’s prescription-drug benefit.

Ordinary Part D does not pay for medicines prescribed for weight loss as of September 26, 2026, though it may cover some other accepted uses. B

The reason for the prescription matters, rather than the ingredient name alone. C

There is a separate temporary Medicare route

Hunger and blood sugar are affected by GLP-1, a signal the body makes naturally. Medicines that copy this signal are also called GLP-1 medicines. D

The Medicare GLP-1 Bridge provided eligible Part D members with certain weight medicines at a monthly copay of $50 as of September 26, 2026. E

The share of a bill that the patient pays is a copay.

The temporary program began July 1, 2026, and is scheduled to end December 31, 2027. E

You must be at least 18, but age alone does not qualify you. The program checks weight and health when GLP-1 treatment began. F

One way to meet the health test is a BMI of 35 or more. Height and weight are used to calculate BMI; ask the clinician to calculate your number. F

With a BMI of 30 or higher, a person may qualify if they have a listed heart-failure diagnosis, blood pressure exceeding the program’s limit even with two medicines, or kidney disease that is long term and at least stage 3a. F

The heart-failure diagnosis must be “heart failure with preserved ejection fraction.” Have your clinician check whether this form of heart failure is named in your records. F

Blood pressure is above the program’s limit when the top reading exceeds 140 or the bottom reading exceeds 90. The readings use mm Hg, a blood-pressure unit. F

Take those exact names and numbers to your clinician. Ask them to match your diagnosis and measurements to the program form.

At a BMI of 27 or higher, a person may qualify with pre-diabetes (blood sugar that is higher than normal yet below the diabetes range), a previous stroke or heart attack, or symptoms from artery disease in the limbs. F

An allowed plan type is also required: for example, standalone Part D drug coverage or a Medicare Advantage plan that includes drug coverage. Ask your plan to confirm its type is included. F

The Bridge cannot be used in 2026 by someone whose GLP-1 medicine was already paid for by Part D that year. The program also needs a prescription, advance approval and ongoing food and activity changes. F

Prescriptions for uses covered by ordinary Part D go to that plan instead. Examples include type 2 diabetes and moderate to severe blocked breathing during sleep in adults with obesity. F

Another is MASH, a liver illness involving moderate to advanced scarring without cirrhosis, the severe form of scarring. A fatty-liver diagnosis alone does not establish that exact condition. F

Ask your clinician and plan to check the full form with you. These notes cannot tell whether your own records meet every requirement.

Ask in your own words

Which health test do my records meet, and does my prescription belong under regular Part D or the temporary Bridge?

The Bridge payment has separate limits

As of September 26, 2026, the $50 Bridge payment did not count toward Part D’s yearly out-of-pocket cap, and coupons could not lower it. G

Out-of-pocket means money you pay yourself. Ask the plan to explain which spending counts toward its limits.

Ask the account administrator about health savings funds

A health savings account is called an HSA; a flexible spending arrangement is called an FSA. Both set aside money for medical expenses.

As of September 26, 2026, weight treatment paid with these funds generally needed a doctor’s diagnosis of the disease being treated; examples are obesity, heart disease and high blood pressure. H

Wanting to improve general health alone does not meet that medical-expense test. H

Have the account administrator explain which records are needed for your expense.
