Check an Arizona doctor’s record

Get the full name, degree and license number. Match the record to the person who will make the care decision.

Use the Medical Board for an MD

MD means medical doctor.

As of October 3, 2026, the Arizona Medical Board’s Doctor Search provides license records and public board orders for MDs. A

A board order is the board’s written decision about a doctor. It may record discipline, such as limits on the doctor’s work. Open it and read what the board decided. B

Ask in your own words

What name and license number will I find in the Arizona Medical Board record?

Use the DO board for a doctor with that degree

DO means doctor of osteopathic medicine, another kind of doctor degree with its own Arizona board. C

As of October 3, 2026, Arizona’s Osteopathic Board has a separate Find your DO search. C

An MD search does not replace that DO search. C

A short online history is not the whole history

Under Arizona rules checked October 6, 2026, only certain board records are shown online; people can request more public records. D

An empty list of actions does not show that there has never been a complaint or an action. D

The five-year limit on online board actions does not apply to legally kept public meeting notices or minutes, the written account of a meeting. D

For an older concern about your doctor, ask the board for the public records it can release. Ask which meeting papers, if any, would help answer your question.
